Salary : £32,306 to £39,027 a year per annum (pro rata)

We have an exciting opportunity for a motivated, enthusiastic and responsible School Nurse SCPHN to work within our 5-19 Healthier Together Team based in Middlesbrough.


Main duties of the job


Responsible for leading and delivering the Healthy Child Programme 5 19 and the 6 school aged years high impact areas, ensuring services are provided within the tiered model approach building community capacity, universal, universal plus and universal partnership plus. To be professionally accountable and responsible and adhere to and apply Trust and NMC standards.

Job description

Job responsibilities


Work with parents, carers, and children and partner agencies to promote the physical, emotional and social development of school age children (includes care leavers and young people up to 25 where additional needs are identified applies to some commissioned contract arrangements).

The post holder will ensure inter-agency/professional partnership working between services, statutory and voluntary organisations to facilitate integrated approaches in meeting the needs of children.

Person Specification

Education/ Qualifications


  • Registered Nurse.
  • Specialist Community Public Health Nurse Qualification (HV)/ SN) at postgraduate diploma level or equivalent.
  • Evidence of continuing professional development.
  • Highly developed specialist knowledge relevant to working with children and families underpinned by theory and experience.
  • Mentorship.
  • Nurse Prescribing.


  • Nurse Prescriber.
