Working as a Specialised Practitioner in Community Public Health Nursing – Health Visiting the post holder will support delivery of the 0-5 Right Start service;

  • Delivery of Healthy Child Programme 0-5 years / stage 1 2 3 and 4 of right start model.
  • Delivery and co-ordination of a range of early childhood services within the designated children’s centres, providing the “front door” for the service
  • Delivery of Children’s Centre Core Purpose

This will be delivered using the national Health Visitor vision and Service Model as a framework to support delivery. “Call to Action” model DH (Feb 2011):

Duties and Responsibilities

  • To take named health visitor responsibility as an independent practitioner for continuous responsibility for a designated caseload identified as Universal, Universal Plus and Universal Partnership Plus.
  • To develop and deliver healthcare and health promotion using evidence based practice, bringing professional leadership to the wider right start team
  • To promote positive health to individuals, families, groups and local communities
  • To contribute to the development of best practice and facilitate shared learning to develop services and initiatives that meet local need.
  • To develop effective working relationships with key stakeholders in delivering services to enable individuals and families to improve their health and wellbeing
  • To appropriately delegate care to members of the multi skilled team ensuring oversight of care delivered and supervision
  • Offering evidence based advice, practice, guidance and high level judgement on the management and care of health needs for the whole family.
  • To facilitate the delivery of clinic and group activities / sessions as required
  • Accountable for own professional practice
  • Accepts delegated line management responsibility for members of the multi skilled team which may include performance and development reviews, guidance  and management supervision
  • In the absence of the team leader will take delegated responsibility for supporting delivery across the team. These duties may include responding to queries from the public, allocation of work, the delegation of duties to wider team members and allocation of team activities
  • .Assessing social circumstances, which may require further support or multi-agency liaison, referral and intervention, identifying risks to the health and well being of children.
  • Identifying children in need and their families and promoting the well-being and safeguarding of children.
  • Identifying risks to the safety of all client groups in a variety of settings, including assessment of own and colleague’s safety before and during home visits.
  • Referral of children in need and participation in the decision making process in cases requiring child protection in accordance with local procedures
  • Monitoring and continually assessing vulnerable and disadvantaged groups or individuals to ensure that the programme care is consistent and adequate for their needs
  • Supporting local Health Improvement Plans  with health enhancing activities in community groups and community settings as appropriate
  • Prescribing within the scope of the Nurse Prescribing Formulary, to support access to healthcare
  • Work with local communities and partner agencies to identify and respond to population and community needs .

The Health Visitor as lead practitioner:

  • Will participate in maintaining high standards of professional care and in setting standards of care appropriate to her/his position.
  • Will welcome and form constructive relationships with clients, general public and professionals in other statutory agencies.
  • Will be fully conversant with and work according to local Child Protection Procedures.  Will complete relevant documentation, prepare and submit professional reports, attend case conferences and Core Group Meetings.  When required will prepare Court reports and attend court as necessary.  To seek the necessary advice, support and supervision from safeguarding nurse Specialist Team members.
  • Will ensure resources are used efficiently and effectively and the quality of service delivered is assured and monitored.
  • Will participate in the development and educational needs of service staff, pre-registration nurse learners and post registration students as required.

The Health Visitor as an Administrative Manager is responsible for:

  • Maintaining contemporaneous and accurate Trust records observing the Data Protection Act 1988, maintaining confidentiality.
  • Maintaining a Trust electronic diary for the purposes of planning work.  Diaries may be requested to be submitted for audit purposes.
  • Reporting to and consulting with the team leader / district manager on matters relevant to the provision of a high quality, safe right start Service, which meets the Trust’s objectives and service specification .
  • Working within the policies of the Trust, adhering to them following guidelines and procedures laid down.
  • Acting as mentor/preceptor to new Health Visiting staff and participate, where appropriate in developing the skills and potential of colleagues.
  • Attending staff meetings and ensuring that minutes are read and signed, especially when attendance has not been possible.
  • Using and developing managerial skills, in relation to assessing, planning, implementing and evaluating care – whether in relation to an individual or a target group within the population.
  • Fulfilling Trust guidance regarding timescales for contact with clients.
  • Participate in personal development reviews (PDRs) and clinical supervision.
  • Attend relevant in-service training sessions
  • Observe the complaints and disciplinary procedures in accordance with Trust policy.
