Family Support Lead

We have an exciting and new opportunity for a passionate and dynamic Health Visitor or Social Care Professional with extensive experience of working with disabled children to join our Community Services team.

Tŷ Hafan is a children’s hospice based in Sully serving the population of South, Mid and West Wales. We offer care and support to children and young people with life limiting illness and their families at the hospice, in the community and in their home. 

The successful candidate will be part of a community team facilitating services and support to children with a life limited condition with the main focus of this role being the facilitation of wider family support services including psychosocial support, sibling support and bereavement support. The aim is to enable children and their families, through appropriate assessment, care planning and intervention, to reach their individual full potential, to have equal opportunities, choices and rights in their own community. They will act as a source of expertise in relation to children’s palliative care / child development, bringing a wider public health approach to the team.

The post holder will be the team leader for the family support staff within the wider multidisciplinary team and manage them on a day to day basis, being responsible for the management of services and processes that support the core business of the organisation. They will work alongside a Play and Therapies Lead who facilitates a hospice and community based service to children and families.

Candidates need to have relevant experience and be confident to lead a multidisciplinary team and work as part of the community management team.
